Program Request Form
Thank you for your interest. Please be aware that completing a request form does not guarantee a volunteer for your classroom. JA Implementation Standards do not allow for teachers to conduct JA programs without a volunteer. Educators new to JA are required to speak with a JA staff member prior to the program start. Once your request is matched with a volunteer, a JA staff member will contact you to provide more details. If you have questions, please contact the JA office at 216-861-8080. Please note: This form works best if completed on the Google Chrome web browser. If you are having issues completing this form, please call the JA office and we will take your request over the phone.
